Job DescriptionScope of PositionIn cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel.
Approves new drug orders, which may include checking/verifying chemotherapy notes, informed consent and processing non-formulary requests.
Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions.
Participates in appropriate hospital and/or departmental committees relevant to practice area.
Position SummaryThis position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas.
Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses.
Patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.
This position is also responsible for supervising Pharm.
D.
students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff).
Duties and ResponsibilitiesDispenses medication pursuant to a physician's order according to the laws and regulations as defined by the Ohio State Board of Pharmacy and The Ohio State University Medical Center's drug distribution policies.
Enters or reviews entry of medication orders into the pharmacy computer system.
Oversees appropriate packaging for drugs including oral, parenteral, ophthalmic, intra-arterial, intra-ocular, topical and investigational medications.
Prepares and supervises the preparation and labeling of medications for dispensing items identified above.
Compounds or supervises compounding of special preparations and parenteral medications.
Maintains patient mediation profiles, which include drug allergies and disease states.
Monitors the accuracy of drug dispensing.
Fulfills all applicable MUE requirements.
Complies with OBRA 90 requirements and accreditation standards applicable to pharmacy practice and employment.
Minimum Education RequiredBachelor's Level DegreeRequired QualificationsRegistered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.
D.
/MS) and residency; and/or equivalent experience preferred; or score a minimum of 40 points on Pharmacy?s Competency Based Advancement worksheet.
